Since 2012, the National Nurse-Led Care Consortium (NNCC) has partnered with Public Health Management Corporation Health Network, made up on five Federally Qualified Health Centers (FQHCs) in Philadelphia to build out sustainable hepatitis C virus (HCV) programming. NNCC has supported the PHMC Health Network to sustain increased HCV screening and treatment program through 2018.
NNCC’s most recent undertaking has been to build on the partnership with the PHMC health centers to support viral hepatitis and opioid use disorder outreach and education programming for underserved communities. Through the relationship with the Philadelphia Department of Public Health, NNCC is now supporting the hepatitis B virus (HBV) surveillance and screening program, with the intention of learning from best practices of the HCV program, including community outreach, screenings, HBV immunizations and linkage to care.
