SDOH Screenings for Patient-Centered Approaches & Care Team Optimization (Three-Part Webinar Series)

In this three-part webinar series hosted by the National Nurse-Led Care Consortium (NNCC) and Louisiana Primary Care Association, Inc, participants can expect to learn about the importance of SDOH screenings and whole-person wellness to improve chronic disease management and outcomes. Presenters will showcase best practices for patient-centered approaches to care by utilizing SDOH screenings and care team optimization and coordination. They will also share their experience executing patient-centered SDOH screening tools and referrals to support services for patients with chronic diseases. Participants will walk away from the series knowing how to best use staff for team-based management.
