In this webinar, participants will hear from lead researchers Dr. Adeline Nyamathi, Dr. Benissa Salem, and Emily Van Cise, Senior Director of Health and Wellness at Downtown Women’s Center and their community-engaged participatory research study that recognized the perceptions of Hepatitis C (HCV) positive patients experiencing homelessness and the design and implementation of the nurse-led community health worker (CHW) team in HCV treatment. The webinar will promote whole-person wellness, and presenters will share the nurse-led CHW team's role in promoting tangible, emotional, cognitive, and behavioral support, and financial and structural resources. Participants will hear from community health workers from the Camden Coalition, who will account real-world implementation strategies, best practices, return on investment, and lessons learned. In addition to this webinar, registrants will gain access to a website that will house additional resources, literature, testimonials from the health centers, and the webinar recording.
