This interactive panel explored how health centers can expand access to cancer screening and improve follow-up care for public housing residents and other medically underserved populations. This session highlighted practical strategies to strengthen the screening-to-resolution pathway, including integrating community health workers (CHWs), patient navigation, and care coordination into team-based care. Panelists also discussed redesigning workflows, leveraging digital tools and health information exchange, and addressing fragmentation to ensure timely diagnostic follow-up and continuity of care.
