Virtual Health Self-Management Education During COVID-19

The Keys to Quality Health Center Controlled Network (a project of NNCC), and the Health Promotion Council are hosting a six-part Project ECHO peer learning series to support the development of HSME programming to be delivered via electronic patient engagement platforms. Health Centers will contribute to didactic sessions and guided case reviews focused on developing HSME content to support health and wellness remotely, and deliver content remotely as we navigate COVID-19 pandemic distancing limitations. This ECHO is free for Keys to Quality HCCN member health center staff. Each module in this ECHO is pending for up to 1.0 prescribed credit hours by the American Academy of Family Physicians. The American Nurses Credentialing Center (ANCC) and other accrediting bodies accepts AAFP CME toward its member continuing education requirements.

Participant Expectations

  • Participate in at least 4 of the 6 live ECHO sessions
  • Submit case presentations to further learning with the team
  • Participate in case discussions with your peers (NNCC will support members with this)
  • Participate in evaluation activities
  • Provide feedback about progress made towards delivery of HSME

ECHO Session Format

Each 60-minute ECHO session will consist of:

  • Didactic (educational) content (~20 minutes)
  • Case presentation (~5 minutes)
  • Participant questions (~10 minutes)
  • Peer recommendations (~10 minutes)
  • Summary of recommendations (~5 minutes)
  • Close and debrief (~10 minutes)
