The National Nurse-Led Care Consortium (NNCC) invites health center clinicians and staff to participate in the Patient-Centered Excellence Learning Collaborative for Elevating Health Center Operations and Quality. Designed specifically for those working in health centers serving vulnerable populations: clinicians, nursing leaders, CHW, care coordinators, and health center administrators, this four-part series will provide actionable strategies to enhance patient-centered care, foster equity, and improve health outcomes.
Through expert-led sessions, engaging peer discussions, and shared insights, participants will explore strategies to prioritize patient experience, implement team-based care, and address cultural and systemic barriers to care. This learning collaborative offers practical tools for integrating patient-centered care into daily operations, knowledge from health equity and community health leaders, and opportunities to connect with professionals committed to advancing health center excellence.
Participants will gain practical insights and strategies to foster a patient-centered approach in health center operations and quality improvement initiatives, ultimately contributing to improved patient satisfaction, health outcomes, and overall healthcare excellence.
During session 3, learn about success stories and tools for inclusive patient engagement and learn how to engage patients as partners through patient and family advisory councils (PFACs) and other innovative strategies.
