Advancing Perinatal Care Learning Collaborative: Promising Practices to Tackle Maternal Mortality - Part One

About the Learning Collaborative

This four-part learning collaborative, Enhancing Perinatal Care Access in Underserved Communities, will equip health center professionals with actionable strategies to improve maternal health outcomes and expand access to quality perinatal care for individuals in low-income housing. This initiative aims to support healthier pregnancies and reduce complications for mothers and infants by focusing on evidence-based practices, care coordination, and workforce development.

The collaborative will highlight individuals' unique challenges in low-income housing, including barriers to healthcare access, environmental factors, and increased exposure to early life stressors. Through expert insights and practical solutions, the series will emphasize strengthening access to comprehensive, high-quality perinatal care, enhancing patient-centered approaches, and addressing factors that influence overall well-being.

Join us as Elisheva Khalfin Elbaz, MS, BSN, RN, RCST, Nurse Home Visitor at the Nurse-Family Partnership at National Nurse-Led Consortium, and Shanna Williams, LCSW, MEd, Perinatal Support Professional and founder of S.W. Doula, LLC, explore the role of maternal and newborn home visitation programs and how birth support professionals contribute to improving health outcomes. Victoria Young, LPC-S, Behavioral Health Counselor at Open Health Care Clinic, will provide strategies for early identification and intervention for mental and emotional well-being during the perinatal period. Samia Bristow, Vice President of Programs at Maternity Care Coalition, will discuss the connection between family safety and maternal health, highlighting approaches to support individuals experiencing challenges during pregnancy and postpartum.

To obtain 1.0 contact hours of continuing professional development in nursing, you must participate in the entire activity, complete the evaluation, and achieve an 80% pass rate on the knowledge gain assessment following the session.

Schedule:

This learning collaborative will occur weekly on four (4) Thursdays from 3:00pm to 4:00p.m. ET (3/11/25, 3/18/25, 3/25/25, & 4/01/25).
