Discover how health centers can turn data into action to improve outcomes for children with chronic conditions. In this session, the Corporation for Supportive Housing (CSH) will showcase how Annual Health Center Needs Assessment data can be transformed into interactive dashboards that reveal chronic disease trends, uncover care gaps, and support strategic decision-making aligned with MAHA priorities, UDS measures, and HRSA expectations—while elevating the patient and family experience. NNCC will then bring the data to life through a real-world case study, demonstrating how nurse-led care teams use health information technology (HIT) and dashboard insights to drive outreach, strengthen follow-up, and deliver coordinated, team-based care that supports quality improvement and HRSA site-visit readiness.
