Embedding Financial Capability Counseling into Health Center Programming

Social determinants of health (SDoH) play a major role in a patient’s ability to achieve and maintain optimal healthcare outcomes. Financial literacy and stability are important determinants of a patient’s ability to access health care and adhere to treatment plans. Providers can adopt partnerships with community organizations to address patients’ financial needs and provide benefits in and outside of the health center setting. This learning collaborative explored strategies to integrate financial capability programming in a primary care setting. This learning collaborative ran from January to March of 2018.
