Join NNCC to discover how enhanced case management in FQHCs strengthens access and affordability by connecting expectant mothers to coordinated prenatal care while addressing barriers such as cost, transportation, provider availability, housing instability, and food insecurity. The sessions will also highlight how this approach fosters patient-directed care and improves maternal and infant health outcomes by strengthening care coordination, integration, and connections to wraparound services and community resources.

National Nurse-Led Care Consortium is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation. This meeting session is pending accreditation for 1.0 contact hour of continuing nursing professional development per session. To obtain nursing continuing professional development contact hours, you must participate in the entire activity, participate in audience polling and/or Q&A sessions, and complete the evaluation.
