Participants will explore practical strategies to address chronic conditions through telehealth and home-based care, while learning about promising approaches to reduce isolation, integrate mental health and community support services, and collaborate with housing authorities to promote safe, healthy living environments.

National Nurse-Led Care Consortium is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation. This meeting session is pending accreditation for 1.0 contact hour of continuing nursing professional development per session. To obtain nursing continuing professional development contact hours, you must participate in the entire activity, participate in audience polling and/or Q&A sessions, and complete the evaluation.
