The National Nurse-Led Care Consortium (NNCC) offered the Safe and Healthy Homes Program (SHHP) from 2016-2021 in Philadelphia and Delaware County in Pennsylvania. The main activity of this program was providing free primary prevention home visiting services to reduce risks associated with unhealthy homes. SHHP strived to reduce illness and injury caused by home health and safety hazards such as fire and fall hazards, asthma triggers, and lead.
This program was funded by the Pennsylvania Department of Health.
NNCC worked with exceptional partners to provide SHHP services, including: Community Action Agency of Delaware County, Delaware County Construction, Health Promotion Council of Southeastern Pennsylvania, Philadelphia Department of Public Health, and Temple University.

Our Impact
Through the work of a committed team, NNCC and its subcontractors reached 1,220 families and provided needed education about injury prevention, lead poisoning, asthma, and other home health hazards that would otherwise not have occurred. By providing over 5,200 low-cost interventions, SHHP was able to assist families in making real change in their homes. Improving the lives and health families through SHHP is NNCC’s proudest achievement.
The Safe and Healthy Homes Program offered comprehensive health and safety education, delivered by a team of trained community health workers—trusted voices from the community.

Although the program has wrapped up, there is a great need for this type of work to continue in communities across the country.
