Erin Graham, DNP, CRNP, FNP-BC, IBCLC, has been appointed as the first-ever Deputy Executive Director at the National Nurse-Led Care Consortium (NNCC), a newly established leadership role designed to strengthen NNCC’s mission of advancing health equity and supporting community well-being through quality, compassionate, and collaborative nurse-led care. Erin’s history within NNCC as a Nurse-Family Partnership (NFP) Nurse Home Visitor, NFP Supervisor, and Senior Director of Healthcare Integration has provided her with comprehensive experience in both direct service and administration. Her journey with NNCC reflects a deep commitment to public health and a strategic vision for expanding access to essential services.
When Erin decided to pursue a Doctorate in Nursing Practice and become a board-certified Family Nurse Practitioner while working at NNCC, she embraced it as a challenging and rewarding opportunity to enhance her clinical and leadership skills to better support her family and community. During her studies, she developed a strong appreciation for the power of data to shape and improve programming. Dr. Graham combines her clinical expertise with an intentional approach to data-driven decision-making, best reflected in her development of NNCC’s case management system. This system has streamlined data collection and reporting, improved workflow efficiencies, and informed quality improvement initiatives.
Erin has also been instrumental in forging collaborative partnerships with healthcare providers, public health agencies, and community organizations to expand access to services for NNCC’s clients. Her ability to build relationships with these external entities has strengthened NNCC’s network of support, ensuring families receive comprehensive care that meets their diverse needs.
Beyond her professional achievements, Dr. Graham brings a personal warmth and passion to her role. She values direct connections with NNCC’s staff and the communities they serve, finding inspiration in listening to people and sharing joyful moments with the youngest members of the community. Her favorite part of the job is attending events, where she has the privilege of connecting directly with families and, of course, getting to play with the babies.
As Deputy Executive Director, Erin will continue to champion initiatives that promote health equity, foster collaboration, and ensure NNCC’s programs meet the evolving needs of the communities they serve. Her dedication, experience, and compassionate approach make her an invaluable leader in NNCC’s efforts to build healthier families and communities through collaboration.
