Nurse-Led Care Survey: Identifying High-Performing Practices

Nurse-led care improves patient outcomes and produces value. Time to prove it!

By the end of 2016, 85 percent of Medicare fee-for-service payments were meant to be tied to quality or value, with 90 percent by 2018. As we continue to see changes to health care delivery and reimbursement, particularly a transition to value-based care, it’s more important than ever to demonstrate the value nurse-led care can bring to these new care management strategies.

National Nurse-Led Care Consortium (NNCC) and the American Association of Nurse Practitioners (AANP) are working together to identify high-performing practices as part of our work on the Nurse Practitioner Support and Alignment Network (NP SAN). Our NP SAN is developing a registry of nurse-led, high-performing practices to demonstrate the critical importance of nursing under value-based reimbursement. The NP SAN will showcase these practices as part of a national campaign with the Centers for Medicare & Medicaid Services (CMS). This is our opportunity to demonstrate the value of nursing when it comes to transforming clinical practice.

A high-performing practice is one that should meet the following criteria:

  • Sets bold aims related to patient health and value and meets those aims
  • Engages patients and families in their care
  • Advances critical aspects of nurse leadership and nurse-led care
