Join the National Nurse-led Care Consortium for this 4-part Learning Collaborative, which will equip health centers with strategies to deliver targeted, accessible care for aging residents and individuals with disabilities in public housing. This approach addresses chronic conditions through methods such as telehealth and home-based care. Participants will explore promising practices to reduce isolation, integrate mental health and community support services, and collaborate with housing authorities to ensure safe and healthy living environments.

National Nurse-Led Care Consortium is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation. This meeting session is pending accreditation for 1.0 contact hour of continuing nursing professional development per session. To obtain nursing continuing professional development contact hours, you must participate in the entire activity, participate in audience polling and/or Q&A sessions, and complete the evaluation.
