It's almost the new year, and we are spending some time reflecting on NNCC and our community! The items below capture just a few of our 2025 highlights! Take a look.

 

NNCC Trainings

The National Nurse-Led Care Consortium hosts and co-hosts different types of training throughout the year. NNCC is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation. In total, NNCC had 53 training sessions accredited for 69 credit hours in 2025.

Philly SPROUT’s goal in 2025 was to train 400 providers for IECMH and related workforce, and they met the goal with 448 attendees. They hosted an in-person Summit, where they launched the training content for Year 2. The SPROUT team was also successful in hosting three live virtual trainings, inviting a large network of providers and the workforce. They also held two live virtual learning collaboratives, which allowed for more engagement and discussion from a smaller, more specific audience.

The PA-NWC held trainings for its Nurse Diversity Council. This year, the team held 3 virtual trainings and 1 in-person training. The topics ranged from Strengthening the Nurse Resident’s Confidence to Combat Workplace Violence to Creating a Robust Nursing Graduate. In total, these trainings had almost 170 participants!

The NTTAP and HCCN Teams hosted a total of 42 trainings.

 

FAIR Program

 

The Family Advocacy and Integrated Resources (FAIR) Project is an innovative partnership that combines civil legal aid services and concrete needs assistance with maternal and child health home visiting programs. The goal is to provide families with comprehensive support that addresses their immediate needs and long-term well-being, ensuring children and parents thrive together. 

Between January 1, 2025 and November 30, 2025, 1,794 referrals were made to the FAIR Project for 938 unique participants. There were 1,940 legal matters (cases and consultations) addressed, and a total of $1,102,003 has been distributed to families through grants and concrete supports from the Family Support Fund.

This year, we hosted a small Giving Tuesday campaign to raise funds for the Family Support Fund, which is a critical resource designed to address urgent, concrete needs that cannot be met through other funding sources. And the campaign isn’t over yet. You can still donate to the fund and help provide urgent financial relief to Philadelphia Families. You can donate to the Fund here.

 

PA-NWC

The PA-NWC team was proud to host the PA-NWC Nurse Leadership Forum Breakfast during the Pennsylvania Organization of Nurse Leaders (PONL) 2025 Annual Nursing Leadership Conference on October 23–24 in Harrisburg, PA. The Breakfast had over 80 attendees. Nurse leaders from across Pennsylvania came together for meaningful roundtable discussions on advancing nursing roles, promoting health equity, and addressing today’s workforce challenges through purposeful collaboration.

CFPA

The Clinical Faculty Preceptor Academy (CFPA) grant supports the training of undergraduate nursing preceptors and clinical faculty in basic tenets of educational theory, student evaluation, complex communications, and strategies for diverse learners as a foundation for teaching in the clinical environment. 

This past year, they presented at many conferences, including but not limited to AACN, SWPONAL, APHN, and a webinar for the Patient Safety Authority. They also piloted the CFPA 101 course at Jefferson, Nemours, and NNCC. The response from nurses was overwhelmingly positive. Experienced and novice nurses serving as preceptors and as clinical faculty felt the course provided them with the tools and confidence to work with students. 

The CFPA is in its fourth year, and this year the big news is that they are disseminating the project nationwide, and they are coming out with a second course, the CFPA 201. The 201 course will be focused on those teaching graduate and doctoral nursing students and will also be a deeper dive into some of the content from 101. They are seeking additional funding to continue to support this important project for the nursing workforce beyond year four, and are excited about the opportunities and connections they've made along the way.

 

Nurse Home Visiting Programs

This year, from January - November 2025, our three home visiting programs, Nurse-Family Partnership, Mabel Morris, and Philly-MontCo Family Center, served 1,019 families and conducted around 8,711 visits. This means that every two weeks, 1,019 families in Philadelphia met with their home visitors to create and work on their goals - whether in development, health, education, or economic self-sufficiency.

Every month, families in the program also had the opportunity to participate in events hosted by NNCC and their program. With events held throughout the year, ranging from learning about healthy food with The Food Trust to celebrating the end of the year with Mister John’s Music, from visits to Smith Memorial Playground and the Philadelphia Zoo, families gathered with their home visitors and community around the city.

Throughout the year, our families could also participate in advocacy opportunities. They joined us at the National Home Visiting Summit in Washington, D.C., and spoke in front of legislators in Pennsylvania and around the nation about the programs and how they have helped shape their lives.

 

AmeriCorps

NNCC’s AmeriCorps programs place members at organizations across Philadelphia and Pennsylvania that work to eradicate poverty through direct community engagement and program management. These programs start in the summer and end the following summer. 

The Community Health Fellowship Program places members at community-based primary care and public health organizations. For the 2024-2025 program year, they had 11 Members, and for the 2025-2026 program year, they have 10 Members.

They also have the AmeriCorps VISTA Program, where members serve for one year at nonprofit organizations and public agencies. For the 2025 program year, they had 7 VISTA’s serve. You can read about these programs and their members here.

 

Community Action Day

On Saturday, October 11, NNCC celebrated our 10th Annual Community Action Day. This was a fun-filled day that is held to purposefully to not only honor all NNCC home visiting families, but to also build our community and promote civil engagement. This year, we welcomed more than 150 attendees.

A piece of civil engagement was inviting local Philadelphia legislators to the event! We had 7 elected officials and/or their staff attend. They were excited to speak with and meet the families they serve, and the families were able to share what they valued in their communities and what they would like to see changed.

More than 30 community partners provided resources and services to families. Resources included mental health services, lactation support, reproductive health education, and much more!

Postpartum Visit Program

The Postpartum Visit Program connects a maternal-child health nurse to individuals who have recently given birth to ensure everything is going well. A nurse conducts a health assessment of the postpartum parent and baby while providing connections to additional resources. From January - November 2025, the Postpartum Visit Program served 1,578 families in Philadelphia.
